WTC’s Mosaic program goes beyond addressing healthcare needs alone. Our commitment is to enrich participants’ lives with a variety of engaging activities designed to maintain and enhance their quality of life. Together, we offer a wide array of activities tailored to specific preferences and requirements, contributing to physical health and happiness.
Our program offers more than just physical health benefits. We focus on personal growth and community engagement, providing opportunities to sustain and expand current abilities and acquire new skills, such as creativity or daily skill improvement, within a supportive and friendly environment.
How to Apply
Step One
Schedule a Tour
Call or email our Mosaic Program Coordinator to schedule a tour and learn more about our facilities and services. We’re here to answer any questions and guide you through the process. This is a great time to talk to the coordinator about program eligibility.
The adventure towards increased independence and happiness begins here, with program hours from 9am to 3pm, Monday through Friday, offering six hours of engaging activities daily.

Mosaic Program Requirements
Accessibility Information: Spaces in our program are limited for individuals who use wheelchairs for mobility.
Requirements: To participate in the Mosaic Program, individuals must meet the following criteria:
Participants in the Mosaic Program must be 18 years of age or older and have medical needs requiring ongoing assistance. Nursing procedures and other medical needs will be assessed during intake to determine ongoing service needs, including those who may also require assistance with personal care. Participants must be able to attend the program as scheduled and demonstrate the ability to function effectively in a 1:3 group setting. There are no restrictions on ambulation as long as other criteria are met, and adaptive equipment is maintained in safe working order for safety purposes. It is essential that all personal equipment used by participants is in good working condition to ensure safety throughout the program.
